For your initial visit, we will ask you to arrive 10 minutes prior to your appointment so that you can fill out a chart with a medical questionnaire.

A full list of current medications, as some medications impact our dental treatment.
Any x-rays taken at a previous office in the past 2 years. X-rays are an important diagnostic tool for our examination. We are happy to reuse current, existing x-rays to save you time and money. However, if these are unavailable to us at your first visit, we may need to take a selection of x-rays to guide our treatment.
Insurance information, if eligible. Please supply the name of the insurance carrier, plus any group, policy, ID or contract number applicable to your coverage so that we may better serve you.
